The following steps explain how to reset your Fortiva credit card account password:
- The registration form can be found at www.fortivacreditcard.com. Please enter your username and email address.
- As an alternative contact point when registering for the service, please use a different Fortiva email address. Then proceed as follows:
- Visit www.fortivacreditcard.com/forgotpw and enter your customer number (usually at the bottom of your bank statement), date of birth, and last four digits of your social security number the information you provided when you registered for Fortiva. The account is associated with a credit card.
- After you have answered your security questions, click on “Send” and enter the code you received by email or text message to verify your identity.
